Outlook forms

I use a custom form that I designed for Outlook 2003 for my contacts. It includes a "Family" tab with spouse, children, and birthday. This is the default form for the contact folder. When I add a contact directly from Plaxo, it uses the old standard form, so I downloaded a macro which changes the form class to my custom form.

However, when I do that, the plaxo-specific add on at the bottom of the contact form disappears. Is there a way to include it again?
