Updated Plaxo Outlook toolbar, & it no longer appears

I just updated the Plaxo toolbar for Outlook 2003 (SP3 - 11.8169.8172), & it will no longer load. I followed the instructions in the FAQ (http://support.plaxo.com/bin/answer.py?answer=130&topic=52) for re-enabling the COM Add-In, but it still doesn't load. When I restart Outlook after checking the box in the COM Add-Ins prompt, the toolbar isn't there, & when I go back into COM Add-Ins & highlight the Plaxo add-in, under "Load Behavior," it says:

Not loaded. A runtime error occurred during the loading of the COM Add-In.

I've also checked under Help > About > Disabled Items, but Plaxo isn't listed there. I've completely un-installed Plaxo, then re-installed this latest version (C:\Program Files\Plaxo\3.6.1.3\plx_out.dll). I've completely restarted Outlook several times, as well as the PC. The Plaxo log even shows activity for today, but no recent activity:

I've used Plaxo for a while, & I'd like to not completely start over from scratch here.
